- Hospice Medical Director
Our hospice medical director ensures that the hospice team is supported and that the care goals set for each patient are achieved, ensuring that every one of the patient’s care wishes is followed.
- Chaplain Services
When a patient is dealing with a critical illness or is nearing the end of life, our hospice chaplain can address the patient’s spiritual needs.
- Skilled Nursing
Our skilled nurses are highly-trained in pain and symptom management. They are available around the clock for any change of condition.
- Assistance with Daily Living Activities
We offer assistance with activities of daily living (ADLs) during Hospice Aides visits.
- Counseling and Community Services
Resource referrals provided by our Social Workers.
- Physical, Occupational and Speech Therapy Services
We offer physical, occupational and speech therapy services as needed.
- Nutrition Counseling
Our dietitians can offer nutrition counseling, ensuring that the patient’s nutritional needs are addressed. They can perform a nutritional evaluation of the patient, offer recommendations for nutrient intake, and educate patients and family regarding hydration/nutrition issues.
- General Inpatient Care
We provide general inpatient care while in a hospice skilled nursing facility as needed for pain control and symptom management, as well as respite for family or caregivers.
- In-Home Continuous Care
We provide in-home continuous care as necessary for crisis situations.
- Bereavement Counseling
Hospice doesn’t end once your loved one passes away. We offer bereavement counseling for family members who are finding it difficult to cope after their loss.
